It is your official disagreement with the issued decision. Your appeal must be submitted no later than 14 days after you have received the decision. You must submit it in a written form and signed, in Polish language or together with certified Polish translation. If you submit the appeal within 14 days period, the decision that you disagreed with will not be final, which means that your case will be still in the process until the appeal procedure is considered.
